Functional neurosurgery is a type of neurosurgery that focuses on the treatment of neurological conditions that affect a person's movement, behavior, and cognition. It involves the surgical modification or alteration of the nervous system in order to improve or restore function.
The neurosurgery field consists of procedures that modify nerve function, from the brain down to the peripheral nerves in the arm or leg. An electrical stimulator on the brain or spinal cord can be used to reduce Parkinson's disease or chronic pain symptoms. This is done by removing an area of the brain that causes seizures, infusing medication directly into the spinal fluid, or removing an area of the brain causing seizures. Functional neurosurgeons treat patients with neurological disorders such as complex regional pain syndrome, epilepsy, essential tremor, dystonia, Parkinson's disease, nerve injuries, spinal cord injuries, multiple sclerosis, trigeminal neuralgia, Tourette syndrome, failed back surgery syndrome, and brain trauma on a daily basis.
There are several different techniques used in functional neurosurgery, including deep brain stimulation (DBS), which involves the implantation of electrodes into specific areas of the brain; transcranial magnetic stimulation (TMS), which uses a magnet to stimulate specific areas of the brain noninvasively; and radiofrequency ablation, which uses heat to destroy tissue in specific areas of the brain.

In deep brain stimulation (DBS), electrodes are implanted in certain parts of the brain. Electrodes produce electrical impulses that regulate abnormal impulses. It is also possible for electrical impulses to affect various chemicals and cells within the brain.
An upper chest pacemaker-like device controls the intensity of deep brain stimulation. Brain electrodes are connected to the device through a wire under the skin.
The electrodes of a spinal cord stimulator are thin wires, and the generator is a small battery pack. Electrodes are placed between the spinal cord and the vertebrae (the epidural space), and a generator is placed underneath the skin, usually near the buttocks or abdomen. Remote controls can be used by patients to send electrical impulses when they feel pain. In addition to the remote control, the antenna is external as well.
Epilepsy surgery may be an option if medicines don't control seizures. The condition is known as medically refractory epilepsy or drug-resistant epilepsy. The purpose of epilepsy surgery is to stop seizures or limit their severity. It is also done to reduce seizure-related deaths, decrease the use of anti-seizure medications, and decrease their possible side effects.
